Despite common belief, Hospice is not about giving up or losing hope, but about providing comfort, care, and support during life’s final journey. Our team of professionals at Family Hospice and Palliative Care is not only committed to the patient, but the entire family. Hospice enters into an individual’s life when they choose to seek comfort care versus curative treatment. Hospice is designed to assist you and your family with each step of your journey as you seek to make the most of each moment with your loved one. Our goal is to focus on your individual needs and develop a plan of care that incorporates your wishes.

Who qualifies for Hospice?
Hospice begins when curative treatment is no longer expected to be effective and
is designed to care for individuals in the last 6 months of their life. Once this time frame has
been identified, Family Hospice and Palliative Care can begin services immediately.

How do I pay for Hospice services?
Individuals are often surprised to find that hospice services are
available to them at little or no charge. Family Hospice accepts payment from Medicare,
Medicaid, and Insurance companies.
The hospice benefit covers costs related to the life-limiting illness, including medications,
medical equipment, medical supplies and the valuable time and expertise of our team of
professionals. In addition, Family Hospice can also provide hospitalization for
symptom management and respite care to offer the caregiver relief.
A financial assessment can be completed for persons not covered under insurance.

Where can I receive Hospice services?
Family Hospice and Palliative Care provides their comprehensive array of services to people in any residential setting, whether it is their own home, a nursing facility or an assisted living complex. In fact, individuals often find with the support of our hospice services they can remain longer in their home or assisted living residence.
To maintain quality care to you and your family at your residence, our service area includes the following
counties: Adams, Allen, Blackford, Huntington, Jay and Wells.
What is Palliative Care?
Family Hospice of Northeast Indiana is proud to announce the addition of our new service, Comfort-Focused Palliative Care.
To accommodate this new service and the growth of our hospice organization, Dr. Roger Lemmen, having over 20 years of pain and symptom management, has united with Family Hospice as the Palliative Care Director.
Palliative Care includes enhancing the quality of life for patients suffering with pain or other symptoms related to their serious or life threatening illness.
Patients at any age and at any stage of advanced illness are eligible.
A physician certification of life expectancy is not required.
Curative treatments may simultaneously be provided with palliative medicine.
Benefits to the patient include guidance and support with difficult medical choices and focus on problematic symptom distress, including anxiety, pain, nausea, breathlessness, depression, fatigue and weakness.
