Family Hospice and Palliative Care is a local, non-profit organization that has been servicing individuals since 1997. Guiding our organization with their expertise, our governing body is committed to ensuring that you and your loved one receive optimal care during life’s journey.

Executive Leadership Team

The Executive Leadership Team oversees the day to day operations of Family Hospice. This highly educated and experienced group of individuals pledges personally to you and your loved ones to ensure our organization brings hope, dignity and comfort to life’s final journey. We encourage you to learn more about our compassionate team in the following biographies.

SueAnn M. Reynolds, BS, MA 
Chief Executive Officer, Family Hospice and Palliative Care

Ms. Reynolds was educated at Indiana University, receiving her BS in criminology in 1991 and her MA in sociology in 1999 from Ball State University. She taught sociology at Ivy Tech State College in Fort Wayne, Indiana for 3 years prior to her involvement in the healthcare field at Caylor-Nickel Medical Center in Bluffton, Indiana. Ms. Reynolds has been the CEO of Family Hospice of Northeast Indiana for the past 8 years. Ms. Reynolds received her hospice administrator’s certification (HACP) in September 2002. Ms. Reynolds has been an IHPCO & NHPCO Professional member since 1999. She is the Chairperson of the IHPCO Education Services Committee for the fifth year and past-Chair of the IHPCO Membership Committee. Ms. Reynolds was the Secretary for the IHPCO Board of Trustee in 2003, the Chair-Elect for two years, and is currently the Board Chair.

Ms. Reynolds was presented the Outstanding Young Women of America award in 1991, International Who’s Who of Professional & Business Women in 1998, International Who’s Who in Professional Management in 1999 and National Register’s Who’s Who in Executives and Professionals in 2002. In 2003 and 2006, Ms. Reynolds was presented with Businessman of the Year from Indiana (a national leadership award) by Congressman Tom Reynolds.

Ms. Reynolds is involved in the community as an Election Board Member and Precinct Committeeman for Adams County, Indiana (positions held since 1989). Ms. Reynolds is a life-long member of the Zion Lutheran Church in Chattanooga, Ohio where she has had the following involvement: Sunday School Teacher, Sunday School Education Superintendent, and Councilman.

Olivia Reeves, BA
Marketing Director, Family Hospice and Palliative Care

Ms. Reeves graduated from Huntington University with a Bachelor of Arts degree in Psychology in 1997. Since graduating from college, Ms. Reeves has served in the long term care industry in various capacities. Ms. Reeves started in the long term care industry as a Social Service Director providing social, emotional, and psychosocial support to residents and their families. Ms. Reeves continued to provide care and expertise to nursing facilities as a Long Term Care Consultant in four states and after consulting, transitioned into admissions and marketing in the long term care setting. Most recently, Ms. Reeves assisted in the opening and growth of a new retirement community.

From a professional standpoint in the long term care setting, Ms. Reeves observed Family Hospice and Palliative Care as a professional and compassionate organization. After serving as a volunteer for the Fundraising Committee with Family Hospice for 2 years, Ms. Reeves learned the exciting opportunity that lied ahead when the position of Marketing Director became available. She readily accepted a position with Family Hospice and Palliative Care.

Ms. Reeves currently serves in the following capacities in the community: Advisory Board Member for the Alzheimers Association of Northeast Indiana, Rotary Club Member, Nominating Committee Chair for the Alzheimers Association, Team Recruitment Committee for Memory Walk with the Alzheimers Association, Leads Group with the Wells County Chamber of Commerce, Membership Committee with the Berne and Wells Chamber of Commerce, Board Member of the Adams County Chamber of Commerce, and Member of the Adams, Berne, Warren and Wells Chamber of Commerce.

Ms. Reeves attends the Life Community Church in Bluffton. She is married to Tony Reeves and has a daughter, Collena. Ms. Reeves states “One of God’s greatest gifts is life and I consider an honor to be involved in the lives of others through Family Hospice and Palliative Care.”

Dr. Bernie Wiebe
Medical Director, Family Hospice and Palliative Care

Born in a rural community in the Canadian province of Saskatchewan Dr Wiebe was raised on a farm with 8 brothers and sisters, and numerous relatives close at hand, including three physicians, sources of career inspiration. Educated at the University of Saskatchewan Dr Wiebe then practiced family medicine in rural Canadian communities for 15 years, including a three year stint volunteering with Mennonite Central Committee in remote Labrador on Canada east coast.

1995 saw Dr Wiebe and family relocating to Bluffton, Indiana, where he joined Markle Medical Center, continuing in the practice of Family Medicine. The following year he assumed the position of Medical Director of Family Hospice of NE Indiana, newly created at that time. He remains in this position.

He has certification in Family Medicine in the College of Family Physicians of Canada. In the US he is board certified in Family Medicine and in Hospice and Palliative Medicine.

He is a member of the board of Bluffton Regional Medical center, where he is co-leader of the Physician Leadership Group (PLG), and where is also chair of the Ethics Committee. In 2002 he was awarded the Charles Caylor Founder's Society award for distinguished and meritorious service rendered to the community.

Cheryl Hoover, RN, BS
Patient Care Director, Family Hospice and Palliative Care

Ms. Hoover was born in Elkhorn, Wisconsin to Yvonne & William Brownlie, the middle of three girls. Ms. Hoover attended Elkhorn Area Schools and graduated from Elkhorn High School as an “Elk” in 1975. Ms. Hoover had always wanted to be a nurse and headed off to Carthage College in Kenosha, Wisconsin for one year and then completed her Bachelor of Science in Nursing from the University of Wisconsin – Milwaukee in 1980. Ms. Hoover started out as an ICU nurse working mostly with open heart patients at St. Mary’s Hospital. Ms. Hoover then married her husband, Dave Hoover, in 1984 and moved to Fort Wayne, IN, where he lived and had grown up. Ms. Hoover had a variety of jobs which included working for an OB doctor, two ICU’s and Home Health, as her and her husband moved around the country with his job. Along the way, Ms. Hoover had 3 children: Kaitlin, Alyse, and Marcus.

Ms. Hoover started working with Family Hospice and Palliative Care over 8 years ago as a PRN nurse, became a case manager, worked as the Quality Improvement Nurse, and now as the Patient Care Director. Ms. Hoover states “God is an ever present guiding force in my life and I believe most things happen for a reason which we may not know for years to come. Often it seems as though the past meets the present because of what we have experienced. Because of my experience with Family Hospice, I was able to care for my sister who was a hospice patient and teach the rest of my family so we could keep her at home. I can relate even more now to what our families are going through as they care for and lose a loved one. That is the case for many of our staff and may even be the reason they came to work at Family Hospice. Our experiences help us to know what our families are going through.”

Ms. Hoover comments that it has been exciting to be at Family Hospice as staff has increased, along with the number of programs offered and patients served. Ms. Hoover is very proud to be a part of the great staff at Family Hospice.

Bonnie C. Herman, BA, MSW, ACSW, LSW
Supportive Care Director, Family Hospice and Palliative Care

Ms. Herman is a native of Decatur, Indiana. She received her BA in Social Work from Valparaiso University in 1971 and her Master of Social Work from Indiana University in 1973. She was employed as a social worker at Bingham Child Guidance Clinic in Louisville, Kentucky for 2 years and then joined the staff at the Mental Health Center at Ft. Wayne, IN where she continued to provide outpatient services to children and families. She received her ACSW in 1976.

Ms. Herman joined the Social Services staff at Family Hospice in June of 2001. She received her LSW in December 2001. She was promoted to Supportive Care Director in February, 2007. She has professional membership in NHPCO, IHPCO and NASW. She serves on the IHPCO Hospice and Veterans Taskforce.

Ms. Herman is a member of Zion Lutheran Church in Decatur where she serves on the Stewardship Board.

Our Team Approach to Care

Family Hospice and Palliative Care provides quality compassionate care to not only you, but your entire family. To reach this expectation, we have several professional teams who are motivated towards meeting every aspect of your care. Whether your concerns are medical, emotional or spiritual, our team is there for you ensuring your needs are being met.

Nursing Team

Once you have made the choice to receive our services, a nurse case manager will be assigned to you to evaluate your needs and develop an individualized care plan based on these needs. Along with the Medical Director, the nurse case manager coordinates your care and assists in symptom and pain management. They also educate the family and/or caregiver by instructing them on how to care for their loved one while caring for themselves. Whether it is the weekend hours or the middle of the night, this team is on-call to meet your every aspect of care so why not call today and allow us to begin this journey together.

Supportive Care Team

The Supportive Care team offers emotional, psychosocial, spiritual and grief support. This team can coordinate community resources, assist with financial arrangements and aid you in preparing your Advance Directives, if needed. When facing end-of-life issues, each of these team members can help comfort and ease the emotional difficulties. Our journey continues with you after your loss by providing you with bereavement services for up to 13 months and will continue longer if needed.

Volunteers

Generously giving of their time and energy, our trained volunteers can assist you and your family in a variety of ways. Some of these ways include providing companionship, assisting with errands, giving caregivers a break or simply just sitting with you to read, listen and offer that support in times of uncertainty. Home Health Aides

The Home Health Aides play a vital role in our team, enabling our patients to retain dignity while assisting them with personal care. These devoted team members assist with household duties to allow the patient and family to continue to enjoy the comforts of home, participate in activities with friends and families, and enjoy walks. While providing support and companionship, one of the biggest compliments our home health aides receive from our patients and families is that they have become a member of the patient’s family throughout this journey.
